Advanced Therapy for Extreme Anxiety Disorders

Anxiety Disorder is the condition wherein the patient cannot process normally their reactions to anxiety-provoking stimuli – including insignificant ones – leading to a panic attack.   Most times the patient is treated with medication and ordered to make changes in their lifestyle to minimize the effect of both symptoms and the severity of their anxiety attacks

When this, too, is insufficient to effect altered thinking in the mind of the patient a therapist will normally recommend what is called Cognitive Behavioral Therapy or CBT.  CBT is beyond the bounds of AMT in that it is psycho-intrusive.   

The goal of CBT is to change how the patient processes the anxiety-provoking stimuli that are now processed inappropriately or pathologically.   

Once the patient has been shown that their processes are flawed and that their reaction to external or imagined stimuli can be altered then the process of indeed altering them can begin.  While it can take a year or more to achieve real demonstrable results, CBT has been shown to produce good results in all but the most extreme of cases.  It is the extreme cases of Anxiety Disorder that we are dealing with here.

To effect changes in the mind of extreme sufferers of anxiety disorder requires repetitive and experimental variation of both techniques and processes of application of those techniques.  This may require the involvement with other professionals to address other conditions like depression which may be present.   Often it is as simple as introducing strong or overwhelming anxiety-producing stimuli and clinically documenting and discussing the resulting affects on the patient.  This role-played introduction of simulated or modeled scenarios is extremely effective because it is real to the patient but not an auto-produced scenario on the part of the patient and many patients requiring advanced therapy respond well to this inside/outside perspective. 

Beyond the basics of CBT lie the relatively uncharted zones of technique that include Formal Therapeutic Modeling, with the introduction of a third party – an actor – who simulates an anxiety-provoking scenario with an appropriate reaction.  This process allows the patient participating in the event to auto-absorb the feelings of the actor – substituting their own for his.

The next technique in the therapists arsenal is called Systematic Desensitization which interrupts or alters the link between an anxiety-producing scenario and the patient’s stored auto-reaction to it. Once that link is broken a new link – to an appropriate auto-reaction mechanism – can be inserted, replacing the aberrant one.  The process is repeated with the next most severe or feared anxiety-provoking issue, source, or stimulus.

Finally, there is a process called Exposure-Response Treatment, or ERT, which causes the patient to be introduced to role-played or simulated scenarios of anxiety-producing stimuli and getting into their reaction and orientation to them.  This ‘getting it out in the open’ process – especially with the Flooding as opposed to the Graduated exposure has shown dramatic results in some extreme cases.  Repeated exposure, with the attendant reduction in reaction, can often produce a neutralized laboratory effect that enables the patient to truly become party to their own response-alteration as opposed to just being a witness or involved observer. The end goal is always to eradicate the disorder and prevent the recurrence of panic attacks
